Thrombus through a patent foramen ovale: a direct cause of cardioembolic stroke and pulmonary embolism.

چکیده
d t R c e We report the case of a 79-year-old man hospitalized for stroke. The patient had hypertension and dyslipidemia and the ECG revealed previously unknown atrial fibrillation. A transthoracic echocardiogram (TTE) was performed two days after admission to exclude an embolic source. This showed marked dilatation of the right cavities, systolic--diastolic flattening of the interventricular septum (IVS) (Figure 1, panel A) and depressed right ventricular systolic function (S′: 8 cm/s; 3D ejection fraction: 18%) (Figure 1, panel D; Video 1). The left ventricle was small and compressed by the bulging IVS; no other wall motion abnormalities were observed. There was a large thrombus 35 mm × 19 mm in size on the right atrium (RA) lateral wall (Figure 1, panel B), and another thrombus 40 mm in length through the patent foramen ovale (PFO) (Figure 1, panel C; Video 2). The patient underwent thoracic CT angiography that confirmed signs of chronic pulmonary embolism and assessed

Stroke prevention in atrial fibrillation and other major cardiac sources of embolism.
The frequency of cardioembolic stroke is expected to rise as the general population ages. Much of the increase may be attributed to atrial fibrillation, the most common cause of cardioembolic stroke and one that plays a substantial role in aging adults. Other sources of cardioembolic stroke may include ventricular thrombus from myocardial infarction, heart failure, structural heart defects such...
متن کاملPatent Foramen Ovale and Brain Infarct
Background and Purpose Paradoxical embolism through a patent foramen ovale is a recognized cause of stroke, but clinical predictors, recurrence rate, and prevention of brain infarcts in patients with patent foramen ovale have not been determined. We reviewed transesophageal echocardiographic records to ascertain echocardiographic predictors and optimal prophylaxis for patent foramen ovale-relat...
متن کاملPost-Operative Multiple Thrombosis Associated with Patent Foramen Ovale: Embolic Stroke, Right Atrial Thrombi, Pulmonary Embolism and Deep Vein Thrombosis
Patients undergoing total joint arthroplasty frequently develop post-operative complication, such as deep vein thrombosis and pulmonary thromboembolism. However, it is not common coexisting deep vein thrombosis, pulmonary thromboembolisms, right atrial thrombus and acute cerebral infarction raised by thrombus through patent foramen ovale. We reported the patient who had multiple thrombi which w...
متن کاملSurgical Treatment for Thrombus Straddling a Patent Foramen Ovale
We present a case of a 41-year-old female with deep vein thrombosis after abdominal surgery. The patient quickly developed severe pulmonary embolism and stroke representative of paradoxical embolism. Echocardiography showed a thrombus straddling a patent foramen ovale, which was confirmed intraoperatively. An accurate diagnosis and rapid treatment decisions are crucial for preventing patient de...
متن کاملPulmonary embolism and patent foramen ovale thrombosis: the key role of TEE
This is a case report of a 35 young man with Klinefelter Syndrome presented breathlessness, palpitations and chest pain. It shows a rare case of a thrombus located through the PFO, in patient with pulmonary and paradoxical embolism, which takes back to exciting hypothesis on thrombus growth. A thrombus, which has grown 'in situ' or trapped through the patent foramen ovale, may be a cause of rel...
